Confidentiality Policy

Purpose Revitalization Inc. is committed to protecting the privacy and dignity of every individual seeking our support. This agreement outlines our standards for handling sensitive information and your rights as a client within our assistance programs.

1. Scope of Confidentiality: All personal, financial, and family records provided during the inquiry and application process are deemed strictly confidential.
2. Authorized Access: Access to client records is restricted to authorized case managers and senior leadership directly involved in the decision-making process.
3. Information Shielding: Electronic data is stored on secure, encrypted servers, and physical files are maintained in locked, fire-resistant cabinets.
4. Limited Disclosure: Personal information will not be shared with outside agencies or sponsors without specific, written authorization from the client.
5. Legal Mandates: Purpose Revitalization is legally required to disclose information in cases of suspected child abuse, elder neglect, or an immediate threat of harm to self or others.

6. Client Anonymity: Success stories and impact reports will never include identifying details (names or exact locations) unless a separate media release is signed.
7. Training and Supervision: All staff and volunteers undergo mandatory ethics training and sign formal confidentiality pledges annually.
8. Electronic Communications: We utilize secure email and document portals for all exchanges involving sensitive application documents.
9. Breaches of Confidentiality: Any unauthorized access or disclosure will be documented, and the affected client will be notified within 48 hours.
10. Retention and Disposal: Files are maintained for seven years post-service completion, after which they are securely shredded and permanently deleted.

By proceeding with an application for assistance, I acknowledge and agree to the following:

I authorize Purpose Revitalization to verify my financial status and application details with relevant utility providers or landlords.
I consent to the collection of demographic and service-need data for the purpose of anonymous grant reporting and organizational improvement.
I agree to receive communications from Purpose Revitalization staff regarding the status of my request via the contact methods provided.
I understand that providing false or misleading information will result in the immediate disqualification from all current and future assistance programs.
